Pharmaceutical brand equity is often treated as a communications outcome, although medicines acquire value through a more complex network of clinical evidence, professional judgment, patient experience, reimbursement, regulation and corporate conduct. This conceptual review integrates foundational brand-equity theory, market-based asset perspectives and pharmaceutical literature to explain how brands create durable commercial and financial value. The synthesis defines pharmaceutical brand equity as stakeholder-held confidence that a therapy and its sponsor will deliver a clinically meaningful, ethically communicated and operationally reliable value proposition. Six interdependent asset domains are proposed: evidence, experience, trust, access, relationships and organizational capability. Together, these assets shape accurate knowledge, confidence, appropriate preference, formulary acceptance, treatment initiation and persistence. Their commercial consequences include launch velocity, access breadth, price realization, lifecycle productivity, selling efficiency and reduced cash-flow volatility. The article argues that awareness and prescription volume are insufficient indicators because they may reflect clinical superiority, patent exclusivity, access rules, promotional pressure or temporary activation rather than incremental brand value. A governance and measurement framework is therefore presented that links leading brand assets to stakeholder responses, market outcomes and risk-adjusted cash flow while preserving patient welfare, fair balance and professional independence. Pharmaceutical firms should manage brand equity as a cross-functional enterprise asset and evaluate it against explicit clinical, ethical and financial counterfactuals.
