The rapid expansion of digital health technologies has transformed healthcare delivery in the United States, offering new opportunities to improve access, efficiency, and quality of care. However, these advances have also exacerbated existing health inequities, particularly among marginalized and underserved populations. This study critically examines digital health equity by synthesizing evidence on disparities in access, utilization, and outcomes associated with telemedicine, mobile health applications, electronic health records, and data-driven health systems. The analysis reveals that structural barriers such as unequal broadband access, gaps in digital literacy, socioeconomic disadvantage, and systemic bias embedded in technological design significantly constrain the equitable distribution of digital health benefits. While digital tools have demonstrated potential to reduce geographic and logistical barriers to care, their effectiveness is uneven and often contingent on broader social determinants of health. The study further highlights governance and policy gaps, including insufficient regulatory oversight, limited community engagement in technology development, and fragmented accountability across public and private actors. Addressing digital health inequities, therefore, requires more than technological innovation; it demands coordinated policy action, inclusive design practices, and sustained investment in digital infrastructure and skills. The paper concludes that advancing digital health equity is essential to ensuring that digital transformation contributes to improved population health rather than reinforcing existing patterns of exclusion.
